Today’s occidental concept of a beautiful nose emphasizes angularity. This type of nose has a straight nasal bridge separated from the forehead by a shallow groove. The width of the nasal bridge is uniform along its entire length. A pleasing nasal tip is narrow and stands out from the straight nasal bridge. The nostrils are thin and gently drop to a narrow nasal base. The angle of the nose from the tip is usually 90 to 100 degrees. The impression of angularity is further enhanced by thin nasal skin. The goal of surgery, the, is to create an esthetically pleasing nose with nasal up projection and angularity in mind, while maintaining facial harmony and balance. Rhinoplasty The first rhinoplasties were performed to reduce large noses. Today, similar procedures are followed to reduce excess amounts of bone, cartilage, and fat in the nose in order to make it more esthetically pleasing. Making the nose larger (augmentation rhinoplasty) to achieve a desired esthetic result has also gained in popularity. This may involve adding cartilage from the nasal septum or from the ear to the bridge and tip of the nose. Silastic (silicone) implants are also frequently used. An augmentation rhinoplasty can help those who are unhappy with an unsuccessful reduction Rhinoplasty therapy. The surgeon is frequently asked to create the same nose as a model’s in a glossy magazine. It is not possible to surgically duplicate a nose. This is because anatomic characteristics differ from one person’s nose to another’s. Before surgery, the physician will ask the patient about any difficulty with nasal obstruction. Intranasal examination may reveal some obstruction may reveal some obstruction to the nasal airway which could be corrected simultaneously in Rhinoplasty therapy In addition, the surgeon assesses facial balance, including the neck chin, upper lip, forehead, cheek-bone areas, and glabella (the area between the eyebrows). A surgical plan based on the esthetic goals is then established and thoroughly discussed with the patient. If the patient is over 50, and wishes to undergo a reduction rhinoplasty, he or she should be aware that there may be an excess amount of nasal skin after the surgery. Due to the loss of elasticity and resiliency experienced with age, the skin does not shrink to the new shape of the nose. Usually this problem is anticipated by the surgeon and is corrected by removing the excess skin or, in some cases, by lifting the brow. What does a rhinoplasty involve? The Steps in Facial Surgery in Chapter 8 apply to a rhinoplasty. Most esthetic rhinoplasties are performed under local anesthesia with intravenous sedation, although general anesthetics are used in some situations. The surgery involves a series of incisions within the nose and occasionally across its base. These incisions enable the surgeon to reach the cartilage, bone, and fat of the nose. Depending on the goal, these tissue components will be modified to produce an esthetically pleasing result. The bone of the nose is often broken as part of the surgery, and this accounts for most of the bruising and swelling around the eyes following the procedure. The incisions are closed with sutures that are absorbed within 7 to 10 days. The nose is often packed with a small amount of Vaseline gauze which is removed the following day. Tape or a plaster-of-paris cast is applied to the nose, and this is removed 7 to 10 days following the surgery. After the intravenous sedation has worn off (within 1 to 2 hours), the patient is sent home with a responsible adult to rest quietly with the head elevated. The following day the packing is removed, either by the patient or the surgeon in Facial surgery. Will the surgery hurt? After the freezing wears off some discomfort is felt in the nasal area for 1 to 2 hours but quickly subsides with oral painkillers (no aspirin). Headaches following the surgery are common but are also managed with painkillers. It is unusual to feel significant discomfort in the nasal area the day following the surgery. After the cast is removed, the nose is tender to the tough for approximately 3 months. Some discomfort may be experienced when the head is bent forward because of pressure due to swelling. As swelling subsides, so will the discomfort.
