Treatment of sun damaged skin and cryothreapy has been used for many years by dermatologists for the removal of age spots and other types of brown pigmented lesions from the skin. 

Recently it has been introduced as a full face peeling agent for the treatment of sun damaged skin.

Nitrogen in its liquid form is very cold and when sprayed on the skin will cause a localized frostbite to the top layer of the skin.  This is known as cryotherapy (cold therapy) in Treatment of sun damaged skin.

The steps involved in a cryopeel are as follows:

A local or topical anesthetic may be used.  If a topical anesthetic is used, it may also be enhanced b other forms of painkillers and muscle relaxants. 

This is important as the procedure is quite painful.  Some physicians use a nonsteroidal anti-inflammatory agent to help control pain.

The skin is cleaned and the face is divided into treatment segments with a marking pen.

The skin in each segment is stretched so the liquid nitrogen can be applied evenly to the surface of each area.

Pigmentary problems and the liquid nitrogen is sprayed onto each segment in a back and forth motion similar to the strokes of paintbrush.

If age spots are encountered more liquid nitrogen is administered to the spots in a spiral like fashion to increase the depth of destruction.

Pigmentary problems and the skin blanches when the liquid nitrogen is first applied. 

This is followed by a period of swelling, blistering and oozing which lasts approximately 5 days.  Subsequent crusting peels off within 10 days.

Once the peeling process is complete the skin is pink in color.  This gradually fades to a normal skin tone over a period of 3to 6 months.

Some physicians will do several very superficial cryopeels where the face is gently sprayed frequently over a period of several months. 

This does not get rid of age spots but does cause superficial swelling which puffs up the fine wrinkles. 

There is some speculation that his process may provide long term benefit by realigning the collagen in the second layer of the skin.  Blistering and oozing does not occur with this technique. 

The skin simply feels tingly when sprayed and may be a pit pink for a short time afterwards.

Does cryopeeling hurt?

The initial contact of the liquid nitrogen with the skin is intensely painful, unless a nerve block or local anesthetic has been used. 

Premedication with a nonsteroidal anti-inflammatory agent reduces this intense pain to seconds after which the patient experiences a dull aching sensation followed by a painless throbbing which lasts approximately half an hour. 

Any further post-operative discomfort is easily managed with over the counter painkillers, although as with other types of surgery acetylsalicylic
acid (aspirin) is not recommended.

What will the face look like after a cryopeel?

The swelling, blistering, and oozing which follows the procedure can be quite unsightly for up to 5 days.  Subsequent crusting is sloughed over the next 5 day period, after which the skin appears pink and smooth and feels tighter. 

Most people take 2 weeks off work because of the post-operative period of cosmetic disability.

What complications may occur?

The complications that may occur with cryopeels are similar to those of other resurfacing techniques and include:

  • Pigmentary problems: Uneven application of the liquid nitrogen may result in a blotchy appearance to the kin.  Damage to the pigment producing cells, known as melanocytes, could cause hypopigmentation might follow inflammatory damage.
  • Scarring:  Overzealous use of the liquid nitrogen may cause damage deep into the second layer of skin which could result in scarring.

Infection:  As with all forms of surgery this is a rare but potential complication

Who performs cryopeels?

Deep cryopeels are not very common because some of the other resurfacing techniques discussed, especially laser resurfacing, provide a better result with less pain and a shorter healing time. 

Superficial cryopeels are more common and are used to freshen the skin and remove age spots.  Dermatologists or other esthetic surgeons trained in the technique would perform cryopeels.
