The patient should be prepared for some discomfort

While the chemical is being applied, particularly with more aggressive agents. 

The pain often disappears within seconds or minutes but may return later, though with less intensity.

Initially, the pain varies in intensity from mild to moderate and is often replaced by itching during the healing process. 

The itching usually subsides after the scabs have come off, although it may persist in some cases. 

Sensations of warmth and tightness are also experienced after a chemical peel in Body care treatments

What will the face took like after a chemical peel?

Sun damaged skin and the extent of cosmetic disability after a peel depends on the depth of the peel.

  Superficial, or ‘freshening peels’, using lower chemical concentrations cause mild inflammation and swelling

Which softens the appearance of wrinkles for a short period of time and may lighten blotchy areas of pigment on the skin in Body care treatments.

Higher concentrations of trichloroacetic acid or of alpha hydroxyl acids, such as glycolic acid, will result in redness, swelling, and, in some cases, oozing. 

Peeling and crusting (if oozing occurs) normally disappear within 5 to 7 days. 

The underlying skin is light pink and gradually fades to normal skin color over a period of several weeks.

  For some patients, a series of moderate peels provides the best results while minimizing the risk factor.

Sun damaged skin and the deeper peels, using higher concentrations of trichloroacetic acid and phenol or its derivatives, cause swelling and oozing of clear or yellowish serum. 

A scab then forms and is usually sloughed off in 1o to 21 days.  The remaining skin is quite red and gradually fades to pink over a 3 to 6 month period in Body care treatments.

When can normal activities be resumed?

After surgery, bed rest with the head elevated is recommended for more aggressive peels. 

If a large area is treated, 48 hours of bed rest is required.  Normal activities at home may be resumed.

But a return to work and social life is not advised until after the crusts have fallen off (5 to 21 days depending on the type of peel).

What complications may occur with chemical peels?

  • Scarring:  Of the complications that may occur, scarring is the most common and cosmetically debilitating.  Scarring occurs more often with deeper than with superficial peels, particularly over the angle of the jaw and around the mouth.  This scarring may result in a band around the mouth resembling a purse string with gathered folds.  Older skin takes longer to heal than does younger skin, but may actually scar less.  Generally, darker skin has a greater risk of scar formation and irregular pigmentation.
  • Hypopigmentation:  Another complication is hypopigmentation, or loss of color, particularly with deeper peels.  The opposite effect, hyperpigmentation, or too much color, may also occur and is more often seen with superficial peels. 
  • Infection:  Secondary bacterial infection or activation of herpes simplex (cold sores) is a potential problem in the areas peeled.  As with other forms of resurfacing, many physicians provide anti-viral medications, such as Valtrex prophylactically in Sun damaged skin.
  • Toxicity:  Internal toxicity is a potential risk with phenol agents causing damage to the heart, liver, or kidneys.  Trichloroacetic acid and glycolic acid (an alpha hydroxyl acid) are not toxic.

Who performs chemical peels?

Dermatologists, plastic surgeons and esthetic surgeons perform mild to aggressive chemical peels depending on the needs of the patient.

Estheticians have recently begun to perform mild chemical peels which give a fresher appearance to the skin but do not dramatically alter the color, texture and wrinkles of the skin.  Unfortunately there are no standards for training or experience for estheticians performing these mild peels.  It would be important to query an esthetician as to their training, competence, and experience in this area.  It would also be wise to ask about the strength and type of chemical being used as well as the potential complications.  Generally speaking if complications do occur estheticians are usually not trained to recognize and correct the problem before significant damage occurs.

Although the peels performed by estheticians are less expensive than those done by medically trained surgeons they are also less effective.  The balance is between risk and benefit.
