The basic steps for brow lift surgery are the same as those outlined in the table on the Steps for Facial Surgery. Three surgical techniques are used, each having distinct advantages and disadvantages. The coronal technique is an incision made either in the scalp or at the junction of the scalp hair and forehead. The mid-forehead technique involves as incision made within the natural creases of the mild-forehead.The direct technique locates the incision just within the hairline of the upper part of the eyebrow. Brow lift surgery and the coronal technique is the most popular because the incision is hidden by the hair. By partially removing some of the muscle as well as the redundant skin, furrows and wrinkles on the forehead can be smoothed out at the same time the brows are lifted. The most up-to-date method of performing the coronal technique utilizes a laser and an endoscope. An endoscope is a small round tube or cannula through which a light, a microscope and the fiber optics of the laser are passed. With conventional scalpel surgery the incision is made from ear to ear across the top of the forehead just within the hair line. The incision is much smaller with the laser and endoscopic technique; 1 to 2 centimeters if reduction of the forehead and glabellar (between the eyebrows) wrinkles is the goal and a bit longer if a brow lift is also being performed. An improved visual field is possible because the laser seals off the blood vessels. The endoscope allows the surgeon to work under the skin without having to pull the entire skin flap away from the forehead in Brow lift surgery. The mid-forehead technique reduces wrinkling on the forehead and lifts droopy eyebrows, however, the disadvantage is that the scar is visible on the forehead. Shapping the eyebrows and the direct method does not reduce the wrinkles on the forehead; it is strictly a technique for lifting the brows. The surgeon can more accurately position the brow with this method, a particularly important consideration when the brows are asymmetrical. Will there be a scar? Shapping the eyebrows and the incision with the conventional coronal technique heals with a cosmetically acceptable, fine white scar which is normally camouflaged within the hair. The scar, however, will be more obvious I balding men and women with diffuse hair loss. At first the scar may feel bumpy and irregular but becomes smooth and flat within 6 months. The incisions with the laser and endoscopic coronal technique are very small and therefore easier to camouflage. In women the incision is usually placed behind the hairline in the middle of the scalp. In men and some women who show signs of male pattern hair loss 2 incisions re made on either side of the head, behind the hair line just above the ears. In this manner the scar is much less obvious in these individuals because hair loss normally takes place on top of the head. Scars from incisions made on the forehead are visible. They are generally pink for 6 months, then gradually fade to a fine white line. The scars from the direct technique follow the same healing pattern but are somewhat camouflaged b the hair of the eyebrows. Troublesome scars can be treated with a combination of injections and laser therapy.
